Severe hypoglycaemia in patients with type 2 diabetes treated with continuous subcutaneous insulin infusion.
Increasing numbers of patients treated with continuous subcutaneous insulin infusion (CSII) to maintain a tight glycaemic control present an atypical clinical picture associated with severe hypoglycaemia, long-lasting or recurrent symptoms, and impaired quality of life. Patients affected by diabetes have a higher prevalence of autonomic neuropathy (AN) and behavioural alterations, with the highest prevalence in those who had already had a severe hypoglycaemic event. AN is associated with more severe hypoglycaemia events and with more difficult glycaemic control. In the present study the authors reviewed the management of a series of 33 patients with type 2 diabetes treated with CSII to analyse their characteristics and to evaluate the frequency of severe hypoglycaemia and the subsequent time spent by patients on insulin pumps. Thirty-three patients with type 2 diabetes treated with CSII from one clinic were evaluated. A questionnaire was administered to patients to collect the following information: presence of severe hypoglycaemia in the previous 12 months and in the previous year; presence of hypoglycaemia-related symptoms of the previous 12 months; maximum recorded blood glucose levels during the previous 3 months; percentage of time in hypoglycaemia in the previous 3 months; and frequency of meals consumed by patients. An analysis was also performed of the percentage of patients treated with bolus insulin plus one or more of the following supplements: oral antidiabetic agents (OAD), CSII, and multiple injections of insulin. The mean age of patients was 48.3 ± 13.9 years and the mean duration of diabetes was 8.9 ± 5.6 years. The number of hypoglycaemia-related symptoms was 0.6 ± 0.5. Severe hypoglycaemia had been reported by four patients (12.1%) over the previous 12 months. The mean blood glucose level was 186.3 ± 45.8 mg/dl and the mean duration of hypoglycaemia episodes was 28.1 ± 8.4 min. The percentage of time spent by patients in hypoglycaemia was 7.0 ± 6.2%. The mean percentage of hypoglycaemia was the highest during nocturnal fasting (9.9 ± 8.9%).
